Form must be submitted at least 1 full calendar month before next billing date for changes to be effective for that billing date. Your account will be cancelled after the next billing cycle. A copy of this cancellation will be emailed to you for your records. You may not request to Cancel if your account is currently on Hold (inactive). You must return to active status for one billing cycle prior to your account being cancelled.
